Using the MyDentalImplantRecords consent form

We know your patients’ peace of mind means yours, too. By having your patient read and sign the comprehensive consent form provided by My Dental Implant Records, you can be sure that they understand how their information is stored and shared. You can access the form right from your patient list.

*Note: The consent form provided by My Dental Implant Records covers electronic communications via the patient portal, email and SMS. If you currently use a patient consent form that covers these forms of communication, this form may not be required.

• From your dashboard, click “My Patients”.

• At the top right of your patient list, click “Disclaimer Form”.

• This will open a preview of the consent form. Click “Print this Form”. This will open a pop-up window with a preview of the form and your printer settings.
• Just print the form, have your patient sign and retain a copy as documentation.
